True Bloom Supports provides specialist Behaviour Support services to children, young people, adults and older adults under the NDIS. Services are tailored to each person's strengths, goals and support needs, with a focus on improving quality of life, increasing independence, building skills, enhancing participation, and reducing restrictive practices.
Functional Behaviour Assessments
Functional Behaviour Assessments help identify the factors influencing behaviours of concern while developing a deeper understanding of the person's strengths, needs, preferences and environment. Assessments guide the development of practical, individualised strategies that support positive outcomes and improved quality of life.
Behaviour Support Plans
Behaviour Support Plans are developed in collaboration with participants and their support networks. Plans focus on building skills, strengthening relationships, improving communication, enhancing participation, and supporting meaningful outcomes while reducing behaviours of concern and restrictive practices.
Restrictive Practice Support
Support is provided to identify, review and reduce restrictive practices in accordance with NDIS requirements. The goal is to promote dignity, choice, safety and quality of life through the use of the least restrictive and most effective supports possible.
Training & Capacity Building
Training and coaching are available for families, support workers, providers and other stakeholders to build confidence and capability in implementing Behaviour Support strategies consistently and effectively.
Ongoing Monitoring & Review
Behaviour Support is an ongoing process. Services include regular review of Behaviour Support Plans, monitoring progress, reviewing incident data and adjusting strategies to ensure supports remain effective and continue to meet the participant's changing needs and goals.
Collaboration & Stakeholder Support
Meaningful outcomes are achieved through strong partnerships. True Bloom Supports works closely with participants, families, carers, support coordinators, providers, schools and allied health professionals to promote consistent, coordinated and person-centred support.
